My Path



My PATH is a health management program that offers you a collaborative team of professionals to guide you every step of the way toward your personal health goals. You will meet with our Nurse Practitioner, Exercise Physiologist and Patient Services Coordinator. The program consists of a combination of individual one-on-one appointments and your choice of facilitated group classes.

Individual appointments: Your first step will be lab work. At your initial medical visit your lab-work results will be reviewed with you by our Nurse Practitioner. At your initial physical activity visit, you will establish health goals that are specific to your needs, preferences, and life circumstances.

You will schedule your mid-program and end-program appointments with the clinicians at your program planning appointment with our Patient Services Coordinator. At this appointment you will pick the class dates and topics you wish to attend. Additionally, you will complete an InBody test. The InBody test provides a comprehensive view of body composition balance – water, muscle, and fat. The test requires you to stand on a scale with bare feet – no socks or shoes. It only takes a few minutes to complete the test and review the results with you.

The InBody test will be repeated at your mid-program and end-program appointments with the clinicians. In total, you will see each clinician 3 times during the duration of your My PATH program. The program will take 4-6 months depending on the classes and dates you choose to attend.

Facilitated Classes: My PATH offers wellness classes to Exclusive Care members throughout the year. There is no limit to the number of classes you may select. The 2018 My PATH Program Schedule appears below.

Do I need a referral for My PATH ?

No referral needed! As an Exclusive Care member, this is a Healthy Highways wellness benefit. There is no copay for any of the appointments or classes and there is no medical criteria you need to meet. However, you must be an Exclusive Care member.

What does My PATH expect from me?

To maximize the benefit of this wellness program, we ask that you complete your lab-work before your medical appointments, keep all of your appointments, attend all the classes you register for, and make a genuine effort towards your health goals. Commitment + Effort = Results. We are with you every step of the way!
